For the
2020-2021 school year only
Most returning athletes that participated in athletics within CISD last year DO
NOT need to turn in a new physical for the 2020-2021 school year. Some
returning athletes might require a new physical or further evaluation based on
answers submitted on this year's medical history form. The medical
history form can be found on the Rank One website.
All students who are incoming 7th graders, new to CISD or new to participating
in extracurriculars at CISD will be required to obtain a new physical for the
2020-2021 school year. Students who are transferring to CISD and
participated in extracurriculars at their previous school should obtain a copy
of their physical from last year from their previous school and turn it in to
their coach at CISD. These students will also be required to fill out the
medical history form on the Rank One website.
In an effort to stream line athletic paperwork, Coppell ISD is offering online
forms. At the beginning of each school year, it is mandatory that all
student-athletes fill out the appropriate UIL and Coppell ISD paperwork before
they will be allowed to practice or tryout.
